The rise of the human genome project and of large-scale genetics studies have spurred a dramatic increase in the number of biobanks in the last decade, increasing their importance in biomedical research.

But until now, biobanks in the U.S. have never been studied systematically, leaving few clear details as to how they are run or the policies and practices they use in managing their work.

A new study from the University of North Carolina published January 25, 2013 in the journal Genome Medicine reveals the huge diversity of U.S. biobanks and also raises questions about the best way to manage and govern them.

"Biobanks are increasingly important to scientific advances, but our decentralized, fragmented research enterprise system in the U.S. has encouraged their development without necessarily providing them with the tools to survive," says study leader Gail Henderson, PhD, professor and chair of social medicine at the University of North Carolina. She also heads UNC's Center for Genomics and Society.

Henderson and colleagues from UNC decided to address this paucity of information by inviting more than 600 biobanks in the U.S. to participate in an online survey. These included private and public, commercial and noncommercial, and many biobanks affiliated with hospitals and academia. Representatives of 456 U.S. biobanks (72 percent of the list invited) participated in the survey.

Among the main findings is their great diversity. "They get established for a variety of reasons; some accidental, some intentional. They vary in size, in when they were established, how formal they are as organizations, what kinds of specimens they hold, who pays for them, and where those specimens come from," Henderson notes.

In the survey, just over half (53 percent) listed research on a particular disease, such as cancer, as the most important reason for establishment. Twenty-nine percent listed research generally. Other reasons included response to a gift or grant, and "intent to centralize, integrate, or harmonize" older specimen collections.

The size of U.S. biobank collections varies, in number of specimens (from tens to millions) and in the types and where they come from individuals, clinics, hospitals, public health programs, and research studies.

Henderson also points out that only a small minority are commercial businesses. "So, not surprisingly, most biobanks do not perceive being in a competitive market. But the majority are quite worried about funding, and many are concerned that the specimens they collect aren't being adequately utilized."

"Researchers and people whose specimens are being held need to be concerned that we don't have a system that is as efficient and effective as it could be," Henderson adds. "If you collect specimens but don't use them, this is a failure to deliver on the promise of advancing translational research, and thus an ethical as well as technical concern."

Biobanks, like the researchers who depend on their services and specimens, need guidance informed by knowledge of their practices and challenges, the authors state. Required are policies "as nuanced as the biobanks themselves," whether these policies address issues of privacy or identity protection, or advancement of research goals.

"Given the diversity in biobank organizational characteristics identified in our survey, it's likely that management and governance policies will have to be tailored to fit the particular context. One-size policies will not fit all," says Henderson.

(AP) ? Heat rising up from cities such as New York, Paris and Tokyo might be remotely warming up winters far away in some rural parts of Alaska, Canada, and Siberia, a surprising study theorizes.

In an unusual twist, that same urban heat from buildings and cars may be slightly cooling the autumns in much of the Western United States, Eastern Europe and the Mediterranean, according to the study published Sunday in the scientific journal Nature Climate Change.

Meteorologists long have known that cities are warmer than rural areas, with the heat of buildings and cars, along with asphalt and roofs that absorb heat. That's called the urban heat island effect and it's long been thought that the heat stayed close to the cities.

But the study, based on a computer model and the Northern Hemisphere, now suggests the heat does something else, albeit indirectly. It travels about half a mile up into the air and then its energy changes the high-altitude currents in the atmosphere that dictate prevailing weather.

"Basically, it changes the flow." said Guang Zhang of the Scripps Institution of Oceanography in La Jolla, Calif. He wrote the paper with Aixue Hu at the National Center for Atmospheric Research in Boulder, Colo.

This doesn't change overall global temperature averages significantly, unlike man-made greenhouse gases that cause global warming. Instead it redistributes some of the heat, the scientists said.

The changes seem to vary with the seasons and by region because of the way air currents flow at different times of the year. During the winter, the jet stream is altered and weakened, keeping cold air closer to the Arctic Circle and from dipping down as sharply, Hu explained.

The computer model showed that parts of Siberia and northwestern Canada may get, on average, an extra 1.4 degrees to 1.8 degrees Fahrenheit (0.8 to 1 degree Celsius) during the winter, which "may not be a bad thing," Zhang said. The effect isn't quite as much in northern North Dakota and Minnesota, where temperatures might be about half a degree warmer (0.3 degrees Celsius), and even less along the East Coast.

In contrast, Europe and the Pacific Northwest are cooled slightly in the winter from this effect. The jet stream changes prevent weather systems from bringing warmer air from the Atlantic to Europe and from the Pacific to the U.S. Northwest, thus cooling those areas a bit, he said.

The biggest cooling occurs in the fall, but Hu said he's not quite sure why that happens.

Several outside scientists said they were surprised by the study results, calling the work "intriguing" and "clever." But they said it would have to be shown in more than one computer model and in repeated experiments before they could accept this theory.

"It's an interesting and rationally carried out study," said David Parker, climate monitoring chief of the United Kingdom meteorology office. "We must be cautious until other models are used to test their hypothesis."

Iraqi extremists try to harness opposition rage

BAGHDAD (AP) ? Iraqi insurgents are trying to capitalize on the rage of anti-government protesters and the instability caused by rising civil unrest, complicating the government's efforts to stamp out a resurgent al-Qaida and other insurgent groups.

Organizers of the protests attracting minority Iraqi Sunnis insist they have no links to terrorist groups. Yet Iraqi and U.S. officials have expressed concern that violent extremists could benefit from the demonstrators' feelings of alienation and hostility toward the Shiite-led Iraqi government.

And tensions are rising.

At least five protesters were killed and more than 20 were wounded on Friday when soldiers opened fire at stone-hurling demonstrators near Fallujah. They were the first deaths at opposition rallies that have been raging around the country for more than a month.

Tens of thousands of protesters turned out in the former al-Qaida stronghold in western Iraq ? some waving black banners emblazoned with the Muslim confession of faith. Two soldiers were later killed in an apparent retaliatory attack for the protesters' deaths.

The vast desert territory on Syria's doorstep was the birthplace of the Sunni insurgency that erupted after the 2003 U.S.-led invasion, and where Iraqi officials believe al-Qaida's Iraq arm is regrouping.

Protesters also have staged demonstrations in other areas with large concentrations of Sunni Arabs who feel discriminated against by the government. Their list of demands includes calls for the release of detainees and an end to policies they believe unfairly target their sect.

For now, the American Embassy said it has no indication that al-Qaida is gaining support from the demonstrations, but the fear remains, particularly as the security situation deteriorates in neighboring Syria.

An embassy official said the U.S. had expressed concern that the protesters' so far peaceful expression of their concerns must not be usurped by extremists trying to provoke violence. The official spoke on condition of anonymity because he was not authorized to speak publicly about the matter.

Protest organizers and the politicians who support them are eager to distance themselves from extremist rhetoric.

Sunni lawmaker Ahmed al-Alawani recently urged Iraq Prime Minister Nouri al-Maliki to meet demonstrators' demands so al-Qaida and other extremists could not exploit their frustration.

That was a sentiment echoed by protest organizer and spokesman Saeed Humaim in Ramadi, the Anbar provincial capital that has been the focus of daily sit-ins and frequent mass rallies. He said protesters have no intention to take up arms, but will defend themselves if attacked by government security forces.

Still, many Iraqi Sunnis have little doubt that the protests strengthen militant groups.

"I don't think the al-Qaida people would miss an opportunity to move freely when the government and security forces are busy handling these spreading protests," said Ayad Salman, 42, who owns a shoe store in northern Baghdad. "The country is slipping toward a new round of civil war, or at least some groups are planning and pushing for this."

The rallies broke out just over a month ago in Iraq's western Sunni heartland of Anbar following the arrest of guards assigned to the Iraqi Finance Minister Rafia al-Issawi, a Sunni who hails from the province.

In an interview aired late Thursday, al-Maliki suggested that al-Qaida and members of Saddam Hussein's ousted regime have a hand in the demonstrations.

"I hope that these protests would not turn violent ... and drag the country to a sectarian war," he told al-Baghdadiya TV.

Al-Qaida's local affiliate this week posted a statement praising the protesters, saluting what it called "the true Muslims who revolted in defense of their honor and religion."

A senior Iraqi security official who specializes in terrorist activities said al-Qaida is making use of the resentment in predominantly Sunni provinces, where local residents who used to provide authorities tips about terrorist activities are growing much more reluctant to snitch.

He and another senior security official said al-Qaida fighters now have more freedom to move around. That is partly because state security forces' movements are being restricted in Sunni areas so they cannot be accused of unfairly targeting the Muslim sect, they said.

The second official said the demonstrations give extremists a good opportunity to try to mobilize Sunni opposition and portray themselves as the only groups who can safeguard the rights and interests of the Sunni minority.

The Iraqi officials insisted on anonymity because they were not authorized to discuss security operations with the media.

The local wing of al-Qaida, known as the Islamic State of Iraq, generally does not operate beyond Iraq's borders. But al-Qaida leader Ayman al-Zawahri last year urged Iraqi insurgents to support the Sunni-based uprising in neighboring Syria against President Bashar Assad, whose Alawite sect is a branch of Shiite Islam.

Iraqi officials believe Sunni fighters aligned with al-Qaida's Iraq franchise are moving back and forth across the Syrian border to help Sunni rebels overthrow Assad.

Rebel gains in Syria are giving Iraq's Sunni protesters and insurgents alike a sense that their fortunes may be shifting too.

"Sunnis seem ascendant in Syria. That is a major psychological boost to the Sunnis in Iraq," said Kamran Bokhari, an expert on Mideast issues for the global intelligence company Stratfor. "They're trying to capitalize on that."

Other militants are trying to tie their fight to the protests too.

Earlier this month, uniformed members of the Naqshabandi Army appeared in an online video urging Iraqis to continue their protests, sit-ins and acts of civil disobedience. It called on security forces to turn their weapons on the "traitors and foreign agents" ? a likely reference to what many Sunnis see is Shiite powerhouse Iran's influence over the government.

The group, a network of former Iraqi military officers and jihadists, frequently claims responsibility for attacks on government security forces.

The highest ranking member of Saddam's regime still at large, Izzat Ibrahim al-Douri, has separately lent his support to the demonstrators. Al-Douri, who is suspected of having ties to the Naqshabandi Army, is thought to have played a key role in financing Sunni insurgents seeking to undermine Iraq's post-Saddam government.

Another small jihadist group, the 1920 Revolution Brigades, put out a statement of its own backing the protest movement.

Earnings season comes but four times a year -- give or take -- and it's upon us again. It's in these mid-season times that we look back on three month's worth of corporate performance and see whose executive strategies are up to snuff and whose, perhaps, need a little retooling.

In the former, happy list we can include Google, whose Q4 2012 earnings added up to $14.42 billion in revenues -- excluding Motorola Home, which would have added another $800 million to the tally. Total profit is a healthy $2.89 billion. We can also happily throw Netflix in the winners group, with its Q4 earnings showing 2 million new US subscribers (for a global total of 33 million) and $8 million profit on $945 million in revenue.


On the losing side we have AMD, which is struggling to keep up with the transition to a post-PC era. Revenues were $1.16 billion, down almost a third from the year before, and resulting in a net loss of $473 million. Logitech too gets added to the losing pile, with revenues down 14 percent to $615 million and a $195 million net loss.

And then we have Apple. The company posted record revenues of $54.5 billion, resulting in a record profit of $13.08 billion, mostly thanks to shuffling an amazing 47.8 million iPhones and 22.9 million iPads into the hands of customers. That all sounds incomprehensibly good, but Mac sales were down nearly a quarter, hurt due in large part to the rather late refresh of the iMac. All in all, Apple failed to live up to analyst expectations and its stock price has been taking a beating -- but that's liable to change by the time you read this.

But, the biggest loser of all is Atari, which filed for bankruptcy protection this week. The brand itself has been sold and re-sold many times since the '80s so it hardly packs the prestige it once did, but still it's sad to see the name struggling on and having to sell off the rights to games like Pong and Tempest to survive.

But enough of that financial stuff, let's talk gadgets.

This is the x86 version of the Surface tablet running full Windows 8 and starting with 64GB of storage. But, given how much of that will likely be consumed by the OS itself, you'll probably want to spring for the $999 128GB model.

Sony's Xperia Tablet Z got official, a 10.1-inch, 1,920 x 1,200 device powered by a 1.5GHz, quad-core Qualcomm processor paired with 2GB of RAM and 32GB of storage. Despite being only 6.9mm thick, it still offers microSD expansion. It weighs only 495g, making it the thinnest and lightest 10-incher on the market. Or, at least, it will be when it actually hits the market, whenever that is.

Nintendo President Satoru Iwata took to the internets to deliver an encouraging batch of news directly to fans, starting with a promised update to overall system speed of the Wii U, a welcome improvement to all those gamers who are short on patience -- which, really, is all of them. The Wii U social network, Miiverse, is also getting a big enhancement, allowing for the creation of more granular groups on there, while also allowing far broader access thanks to a browser interface that will launch this year. Nintendo is also promising that a mobile app will launch sometime this year, which means you can get your fix of obscene stylus drawings wherever you may be.

Finally, we got what may be our first hint at how the next-generation PlayStation home console is coming together, courtesy of a leaked developer document handed to Kotaku. The console is reportedly codenamed "Orbis" and will offer an eight-core CPU and R10XX GPU, both from AMD. If these specs pan out, that's an interesting development, as AMD also powers the Wii U and is rumored to be powering the next Xbox as well.

What was wrong with me? Outside the medical sphere, I am prone to impatience, candor and bouts of argumentative fervor. Had feminine socialization kicked in? As a girl, I was trained to be courteous to people in positions of authority and to revere the saving knowledge of physicians. But men also exhibit symptoms of the good patient syndrome.

Indeed, Anatole Broyard preached its virtues in his book ?Intoxicated by My Illness,? although his version was less compliant, more ironic than mine. ?If a patient expects a doctor to be interested in him, he ought to try to be interesting. When he shows nothing but a greediness for care, nothing but the coarser forms of anxiety, it?s only natural for the doctor to feel an aversion.? Following this logic, Broyard embarked upon an impersonation: ?I never act sick. A puling person is not appealing.? He therefore set out to charm his physicians ? to distinguish himself from boring, easily forgotten patients. I did this too, adding a pinch of obedience, a dash of gratitude, and a smidgen of eccentricity to the mix. One doesn?t want to be just any old patient; patients are replaceable.

Since illness had never intoxicated me, why was I behaving like Broyard? The short answer is terror: these people could hurt me.

Were I to seem boring or easily forgotten, should I appear crabby or disagreeable, I might get neglected or, in my anxious imagination, harmed. Not consciously neglected or intentionally harmed, of course, because doctors and nurses have dedicated themselves to helping people whose sickness often makes them boring and disagreeable. But neglected or harmed nonetheless. Like most patients, I am keenly aware that the medical staff at most facilities are overloaded. It is easy to get left for hours unattended on a gurney or starved and freaked when surgeries are perpetually postponed or distressed and bruised when the bindings on limbs are roughly or hastily applied.

But of course adopting the role of model patient does not provide a solution. Much of the caretaking in hospitals remains out of the control of our personal physicians and nurses. And in any case, too much ingratiating docility can be dangerous to a patient?s health.

If I had persisted in asking my surgeon about the fate of the stents that he had implanted in my body, he might have remembered to remove them. If I had not followed to the letter the dosage he prescribed of a heavy-duty antibiotic, especially as I began to get sick to my stomach and dizzy, I might not have had the full-body breakdown of an allergic reaction. Earlier still, if I had insisted on better bowel preps before my first abdominal surgery, or a postponement, maybe the stents and infection and the allergic reaction to antibiotics would never have happened.

Even before that, if I had challenged my general practitioner who diagnosed indigestion, maybe my cancer would have been found at an earlier stage. If my grandmother had wheels, she?d be an omnibus: that?s a family joke.

So much for the magical thinking that good patients receive the best care. Being a submissive or dutiful patient doesn?t always pay off. Who exactly was I being good for? Sometimes it?s good to be bad.

Was I good for nothing? When I was at my most puling and unappealing and too sick to be good, with pain so overwhelming that I had to be taken to my oncologist?s examining room in a wheelchair, she placed her hand on my knee and kept it there while explaining how she would take care of me. Though I could not look her in the eye, though I could not speak for groaning, I took her point. I had foisted the good patient role on myself. She had always seen through the pose to the mortally sick human being. Why else would I be here, I realized.

At that moment I resolved to renounce or rectify my goodness. I don?t always succeed.

China's Smaller Cities Are Home to Growing Middle Class - Michael ...

Last week Chen Demin, China's commerce minister, offered an upbeat outlook for China's overall 2013 economy, forecasting steady improvement in domestic consumption and investment, and predicting that foreign direct investment over the coming year would also remain stable. In his remarks, Chen emphasized that foreign investors should look favorably upon the opportunities in China, notably those connected to China's construction of many new small and medium-sized cities and towns.

We agree. Robust new construction in China and a soaring rate of urbanization promise unprecedented levels of consumption and overall GDP growth that should only invite greater confidence among foreign investors.

As we stress in The $10 Trillion Dollar Prize, urbanization is one of the most powerful transformative economic forces in China today. According to the National Bureau of Statistics of China, 51 per cent of China's population, or 691 million people, are currently living in urban areas. Research suggests that by 2020, some 824 million people will be living in cities, an increase of 188 million. That's one and a half million new urban residents every month for the rest of this decade. By 2030, according to our analysis, there will be around 270 million more new urban residents in China.

But the distribution of China's urban population is vastly different from that of many other nations, with the majority of that population located in midsize cities ranging between 500,000 and 5 million people.

Typically, and perhaps not surprisingly, U.S. and European companies entering China have focused on the cities of Beijing, Shanghai, and Guangzhou. But we tell them that this is a mistake. There are pockets of wealth all across the country, and the wealth is spreading deep into the country's western provinces.

By our calculations, a company had to be in 60 cities to reach 80 percent of the country's middle class in 2005. Today, they have to be in 340 of them. And by 2020, they will need to be in 550 urban locations to reach that same percentage of the middle class population.

We regroup the cities of China into a series of four categories, including megacities, cluster capitals, specialist hubs, and horizon towns, as follows:

  • Megacities. China boasts two megacities ? Beijing and Shanghai ? each with more than 10 million residents. There are also eight cities with more than 10 million people, and another 93 cities with more than 5 million people. To put this into context, the U.S. has only one city with more than 5 million people ? New York.
  • Cluster Capitals. These include Changshu, Daqing, Dongguan, Fuzhou, Tianjin, Wuxi, and Zhengzhou. These are trade hubs surrounded by smaller "satellite" cities. Wuxi, for example, is the capital of a group of six cities within a 30-mile radius in Jiangsu province: Jingjiang, Zhangjiangang, Changshu, Jiangyin, Changzhou, and Suzhou. On its own, Wuxi has a population of 2.3 million people, including 572,000 middle class consumers. Treated as part of a cluster, however, it becomes the center of a market with 6.9 million people and 1.5 million middle class consumers.
  • Specialist hubs. These are cities whose growth is often closely linked to the development of local natural resources or industrial hubs. These include Anyang, Bozhou, Chengdu, Lu'an, Suizhou, Xinxiang, and Yongzhou.
  • Horizon towns. Finally, there are also hundreds of small, geographically dispersed emerging-market cities. Although hard to reach, they offer ripe market opportunities ? consumers in these towns usually have more basic needs than those of their counterparts in the bigger cities, but they also have a strong willingness selectively to "trade up" to pricier luxury goods.

And take note of one other very promising related trend: as reported by The Wall Street Journal last month, China's droves of migrant workers (reportedly 252 million strong in 2011) who journey from the country's remote rural areas into cities to offer their labor to construction and manufacturing firms are increasingly remaining in those cities for longer periods of time. If this phenomenon continues ? and especially if China's new government relaxes the household registration policy (known as "hukou") that for years has forbidden workers from taking advantage of local benefits (including schools for their children) ? these workers will constitute an ever more powerful new urban work force, and a formidable contingent of newly affluent consumers. As they are permitted to participate in the local social welfare system of the cities where they've relocated, these workers will feel less pressured to save every yuan they earn. They will feel newly emboldened to spend their income on a broad range of products and services, contributing to what we believe will amount to some $6.2 trillion in annual consumer spending in China by 2020. And that's a potential milestone that few foreign investors should ignore.
